Use this form to submit a concern about an organisation not meetings Queensland's Child Safe Standards.

The Queensland Family and Child Commission's role is to promote and oversee child safe organisations. 

Reports submitted through this form should focus on concerns about an organisation's child safe culture, governance, systems, or practices rather than a single incident involving an individual.

The most helpful information describes how the organisation prevents harm, responds to concerns, listens to children, manages risks, supports reporting, and meets its child safe obligations. Where concerns relate to a broader pattern, practice, or organisational response, please include as much detail as possible.

This information will assist us to assess potential child safety risks and systemic issues within the organisation.
