When a child known to the child protection system dies, the system must learn from the tragedy and make the necessary improvements to help prevent future deaths.

The Child Death Review Board was established on 1 July 2020 to carry out systems reviews following the deaths of children known to the child protection system. Under section 29A of the Family and Child Commission Act 2014 (Qld), the purposes of the Board's reviews are to: 

  • identify opportunities for continuous improvement  in systems, legislation, policies and practices
  • identify preventative mechanisms to help children  and prevent deaths that may be avoidable.

This report outlines the activities and finding  from the system reviews it conducted during the Board's first year.
